超重和肥胖儿童并发性早熟的危险因素研究

摘要:

目的:通过对超重和肥胖并发性早熟儿童的临床特征和饮食及生活方式的分析,探讨超重和肥胖儿童并发性早熟的危险因素。方法:选取92例性早熟的超重、肥胖儿童为研究对象,测量体格生长指标,检测性激素水平和代谢指标,采用调查问卷询问父母喂养和养育方式及生活习惯等,以同期就诊于徐州医科大学淮安临床学院门诊的42例性发育正常的超重、肥胖儿童作为对照组。结果:(1) 超重、肥胖并发性早熟儿童体重指数(BMI)、BMI-标准差评分(SDS)、甘油三酯及总胆固醇较对照组低,而胰岛素样生长因子1(IGF-1)-SDS较对照组高,差异具有统计学意义(均P<0.05);(2) 中枢性性早熟(CPP)组的2岁内进食豆制品、母乳喂养≤6月、开灯睡觉及母亲初潮≤13岁占比高于对照组,差异具有统计学意义(均P<0.05);(3) 母乳喂养≤6月及母亲初潮≤13岁是超重、肥胖儿童并发性早熟的独立危险因素(P<0.05)。结论:BMI、BMI-SDS、甘油三酯及总胆固醇与超重、肥胖儿童性早熟的发生并非正相关关系,改变婴幼儿的喂养和生活方式对预防超重、肥胖儿童并发性早熟有临床价值。

Objective: To explore the risk factors of precocious puberty in overweight and obese children through the analysis of clinical features, diet and lifestyle. Methods: A total of 92 overweight and obese children with precocious puberty were studied. Physical growth indexes, sex hormone levels and metabolic indexes were measured. The parents of overweight and obese children were asked about feeding and rearing style and living habits, and 42 overweight and obese children with normal sexual development were taken as control group. Results: (1) body mass index(BMI), BMI-standard deviation score(SDS), triglyceride and total cholesterol of overweight and obese children with precocious puberty were lower than those of the control group, the insulin-like growth factor 1(IGF-1)-SDS was higher than those of the control group,the differences were statistically significant(all P<0.05);(2) The proportions of eating soy products within 2 years, breast-feeding ≤6 months, sleeping with light on and menarche ≤13 years old in central precocious puberty(CPP) group were higher than those in control group, and the differences were statistically significant(all P<0.05);(3) Breast-feeding ≤6 months and menarche ≤13 years old were independent risk factors for premature puberty in overweight and obese children(P<0.05). Conclusion: BMI, BMI-SDS, triglyceride and total cholesterol are not positively correlated with the occurrence of precocious puberty in overweight and obese children. It is of clinical value to change infant feeding and life style to prevent precocious puberty in overweight and obese children.
