胃癌组织GLDC蛋白表达及其与根治术后复发的关系探讨

摘要:

目的:探讨胃癌组织中甘氨酸脱羧酶(GLDC)蛋白的表达及其与根治术后复发的关系。方法:选取在本院行根治术治疗的胃癌患者106例,检测并比较胃癌组织与切缘正常组织中GLDC蛋白的表达水平,分析GLDC蛋白表达与临床病理特征的关系。患者行根治术后随访3年,根据随访期内是否复发分复发组与未复发组,比较2组胃癌组织GLDC蛋白表达情况,采用Cox回归分析根治术后复发的影响因素,并采用Kaplan-Meier生存曲线分析胃癌组织GLDC蛋白不同表达水平患者的复发率。结果:胃癌组织GLDC蛋白低表达率高于切缘正常组织(P<0.05),肿瘤直径≥5 cm、lauren分型为弥漫型、T3~T4分期 、N1~N3分期患者中癌组织GLDC蛋白表达水平更低(P<0.05)。胃癌根治术后3年复发率为65.05%,复发组肿瘤直径≥5 cm、T3~T4分期、N1~N3分期、胃癌组织GLDC蛋白低表达占比均高于未复发组(P<0.05),行术后辅助化疗占比低于未复发组(P<0.05),均为胃癌患者根治术后复发的影响因素(P<0.05)。Kaplan-Meier生存曲线分析显示,GLDC蛋白低表达组的复发率高于高表达组(P<0.05)。结论:胃癌组织中GLDC蛋白表达水平降低,与肿瘤大小、组织学分型、T分期、N分期均有关,胃癌根治术后复发率高,胃癌组织GLDC蛋白表达水平可影响患者根治术后的复发率。

Objective: To investigate the expression of glycine decarboxylase(GLDC) protein in gastric cancer tissue and its relationship with recurrence after radical resection. Methods: 106 patients with gastric cancer treated by radical resection in our hospital were selected. The expression levels of GLDC protein in gastric cancer tissue and normal tissue of incisal edge were detected and compared, and the relationships between the expressions of GLDC protein and the clinicopathological features were analyzed. Patients were followed up for 3 years after radical resection, and they were divided into recurrence group and non recurrence group according to recurrence or not during the follow-up period. The expression of GLDC protein in gastric cancer tissues were compared between the two groups. The influence factors of recurrence after radical resection were analyzed by Cox regression, and the recurrence rate of patients with different expression levels of GLDC protein in gastric cancer tissue was analyzed by Kaplan-Meier survival curve. Results: The low expression rate of GLDC protein in gastric cancer tissue was higher than that in normal tissue at the incisal edge(P<0.05). The expression of GLDC protein in gastric cancer of patients with tumor diameter≥5 cm, lauren classification of diffuse type, T3-T4 stage and N1-N3 stage were lower(P<0.05). The recurrence rate in 3 years after radical gastrectomy was 65.05%. In the recurrence group, the proportions of patients with the tumor diameter≥5 cm, T3-T4 stage, N1-N3 stage and the low expression of GLDC protein in cancer tissues were higher than those in the non recurrence group(P<0.05), and the proportion of patients with postoperative adjuvant chemotherapy was lower than that in the non recurrentce group(P<0.05), and they were all influence factors of recurrence of patients with gastric cancer after radical resection(P<0.05). Kaplan-Meier survival curve analysis showed that the recurrence rate of GLDC protein low expression group was higher than that of high expression group(P<0.05). Conclusion: The expression level of GLDC protein decreases in gastric cancer tissues, and it is related to tumor size, histological type, T stage and N stage. The recurrence rate of gastric cancer after radical resection is high, and the expression level of GLDC protein in gastric cancer tissue can affect the recurrence rate of patients after radical resection.
