复发性流产患者绒毛组织HIF-1α、VHL蛋白的表达及其临床意义

摘要:

目的:检测复发性流产患者绒毛组织缺氧诱导因子-1α(HIF-1α)、希佩尔林道抑癌基因(VHL)蛋白表达水平并分析其临床意义。方法:选取2019年5月至2021年11月本院收治的83例复发性流产患者作为观察组,另选取80例行人工流产的正常早孕者设为对照组。收集两组绒毛组织,采用免疫组化法检测绒毛组织HIF-1α、VHL蛋白表达水平,采用Spearman法分析复发性流产患者HIF-1α与VHL蛋白表达的关系,并采用Logistic回归分析复发性流产的影响因素。结果:观察组绒毛组织HIF-1α蛋白阳性表达率高于对照组(P<0.05),VHL蛋白阳性表达率低于对照组(P<0.05);观察组HIF-1α蛋白表达与VHL蛋白表达呈负相关(r=-0.504,P<0.05);观察组孕妇被动吸烟、人工流产≥3次、作息不规律占比均高于对照组(P<0.05);经Logistic回归分析可知,人工流产≥3次、作息不规律及HIF-1α蛋白阳性表达率升高均是发生复发性流产的危险因素(P<0.05),VHL蛋白阳性表达是其保护因素(P<0.05)。结论:复发性流产患者绒毛组织中HIF-1α蛋白表达水平异常升高,VHL蛋白表达水平异常降低,且两者呈负相关,均可能与复发性流产的发生有关。

Objective: To detect the expressions of hypoxia inducible factor-1 α(HIF-1α) and Von Hippel Lindau tumor suppressor gene(VHL) protein in villi of patients with recurrent abortion and analyze their clinical significance. Methods: 83 patients with recurrent abortion treated in our hospital from May 2019 to November 2021 were selected into observation group, and 80 normal early pregnant patients with induced abortion were selected into control group. The villi tissues of the two groups were collected, and the expressions of HIF-1α and VHL protein in villi were detected by immunohistochemical method. The relationship between HIF-1α protein expression and VHL protein expression in patients with recurrent abortion was analyzed by Spearman method, and the influencing factors of recurrent abortion were analyzed by Logistic regression. Results: The positive expression rate of HIF-1α protein in villi of the observation group was higher than that of the control group(P<0.05), and the positive expression rate of VHL protein was lower than that of the control group(P<0.05). The expression of HIF-1α protein in villi of patients with recurrent abortion was negatively correlated with that of VHL protein(r=-0.504, P<0.05). The proportion of passive smoking of pregnant women, induced abortion ≥ 3 times, irregular work in the observation group were higher than those in the control group(P<0.05). Logistic regression analysis showed that induced abortion ≥ 3 times, irregular schedule and high positive expression rate of HIF-1α protein were the risk factors for recurrent abortion(P<0.05), and the positive expression of VHL protein was the protective factor(P<0.05). Conclusion: The expression of HIF-1α protein in villi of patients with recurrent abortion is abnormally increased, and the expression of VHL is abnormally decreased. The expression of the two proteins is correlated, which may be related to the occurrence of recurrent abortion.
