血清TRAF6、Talin1水平对子痫前期孕妇不良妊娠结局的预测价值

摘要:

目的:探究子痫前期孕妇血清中肿瘤坏死因子受体相关因子6(TRAF6)、踝蛋白-1(Talin1)的表达水平,以及评估两者对不良妊娠结局的预测价值。方法:收集2020年1月至2022年1月在本院接受治疗的94例子痫前期孕妇作为子痫前期组,另选取同时间健康的70例孕妇作为对照组。将子痫前期组根据妊娠结局分为不良组和正常组。通过实时荧光定量PCR (qRT-PCR)检测各组血清TRAF6、Talin1的相对表达水平;并对子痫前期孕妇不良妊娠结局的影响因素进行单因素和多因素分析;绘制受试者工作特征(ROC)曲线分析血清TRAF6、Talin1水平对子痫前期孕妇不良妊娠结局的预测价值。结果:与对照组相比,子痫前期组TRAF6水平和Talin1相对表达量显著升高(P<0.05);且子痫前期组妊娠结局总不良率显著高于对照组(P<0.05)。单因素分析结果表明,血压、24 h尿蛋白定量、TRAF6水平和Talin1水平均是影响子痫前期孕妇不良妊娠结局的因素(P<0.05);多因素Logistic回归分析结果显示,血清TRAF6、Talin1高表达是影响子痫前期孕妇不良妊娠结局的危险因素;ROC曲线结果显示,血清TRAF6和Talin1预测子痫前期孕妇不良妊娠结局曲线下面积(AUC)分别为0.835、0.872,当两者联合时其AUC为0.931,高于两者单独预测(P<0.05)。结论:子痫前期孕妇血清TRAF6、Talin1水平显著升高,与子痫前期孕妇不良妊娠结局密切相关,两者联合检测对预测子痫前期孕妇不良妊娠结局具有较好的效能。

Objective: To investigate the expression levels of tumor necrosis factor receptor-associated factor 6(TRAF6) and Talin1 in the serum of pregnant women with preeclampsia, and to evaluate their predictive value for adverse pregnancy outcomes. Methods: From January 2020 to January 2022, 94 pregnant women with preeclampsia who received treatment in our hospital were allocated into preeclampsia group, 70 healthy pregnant women control group. The preeclampsia group was divided into normal group and abnormal group in terms of the pregnancy outcome. The relative expression levels of serum TRAF6 and Talin1 in each group were detected by real-time fluorescence quantitative PCR(qRT-PCR) method; univariate and multivariate analyses were performed to analyze the influencing factors of adverse pregnancy outcomes in pregnant women with preeclampsia; receiver operating characteristic(ROC) curve was drawn to observe the predictive value of serum TRAF6 and Talin1 levels for adverse pregnancy outcomes in pregnant women with preeclampsia. Results: Compared with the control group, the relative expressions of TRAF6 and Talin1 in the preeclampsia group were obviously increased(P<0.05); and the total adverse pregnancy outcome rate in the preeclampsia group was obviously higher than that in the control group(P<0.05); the results of univariate analysis showed that blood pressure, 24-hour urine protein quantification, TRAF6 and Talin1 levels were all the factors affecting the adverse pregnancy outcomes of pregnant women with preeclampsia(P<0.05); multivariate Logistic regression analysis showed that the high expression of serum TRAF6 and Talin1 was a risk factor for adverse pregnancy outcomes in pregnant women with preeclampsia; ROC curve results showed that the area under the curve(AUC) of serum TRAF6 and Talin1 for predicting adverse pregnancy outcomes in preeclampsia pregnant women was 0.835 and 0.872, respectively, and when the two were combined, the AUC was 0.931, which was higher than that of the two alone(P<0.05). Conclusion: The expression levels of TRAF6 and Talin1 in the serum of pregnant women with preeclampsia are obviously increased, which are closely related to the adverse pregnancy outcomes of the pregnant women with preeclampsia. The combined detection of the two well predicts adverse pregnancy outcomes in pregnant women with preeclampsia.
