血清环状RNA circTRAF3与局部进展期鼻咽癌患者放疗敏感性的关系

摘要:

目的:探讨血清环状RNA circTRAF3与局部进展期鼻咽癌(NPC)患者放疗敏感性的相关性。方法:选取2017年9月至2021年5月在海南省肿瘤医院完成放疗的141例局部进展期NPC患者作为研究对象。采用实时荧光定量聚合酶链反应检测血清circTRAF3水平,并收集随访数据。根据WHO制定的实体瘤疗效评价标准评估放疗完成后3个月的肿瘤反应。结果:局部进展期NPC患者基线血清circTRAF3表达水平为1.93(1.47,2.40),与主要的临床特征无关(P>0.05)。根据基线血清circTRAF3中位值,将NPC患者分为低表达组和高表达组。放疗后血清circTRAF3表达水平明显升高(P<0.05),其表达水平随着放疗次数的增加而逐渐增高。放疗抵抗组基线血清circTRAF3表达水平显著高于放疗敏感组(P<0.05)。Logistic回归分析显示,基线血清circTRAF3水平是局部进展期NPC患者放疗敏感性的独立预测因子(P<0.05)。血清circTRAF3预测局部进展期NPC患者同步放化疗(CCRT)敏感性的受试者工作特征曲线下面积为0.825(95%CI:0.748~0.902)。基线血清circTRAF3高表达组术后无进展生存期、总生存期、远处无转移生存期和局部无进展生存期均显著短于低表达组(P<0.05)。结论:circTRAF3可作为局部进展期NPC良好的预后生物标志物,并可能通过调节患者的放疗敏感性影响放疗疗效和生存预后。

Objective: To investigate the correlation of serum circTRAF3 with radiosensitivity in locally advanced nasopharyngeal carcinoma(NPC). Methods: A total of 141 patients with locally advanced NPC who completed radiotherapy in Hainan Cancer Hospital from September 2017 to May 2021 were selected as the research subjects. Serum circTRAF3 levels were detected by real-time fluorescence quantitative polymerase chain reaction, and follow-up data were collected. The tumor response was evaluated 3 months after the completion of radiotherapy according to the response evaluation criteria in solid tumor established by WHO. Results: The baseline serum circTRAF3 expression level of patients with locally advanced NPC was 1.93(1.47,2.40), which was not associated with the main clinical features(P>0.05). According to the baseline circTRAF3 median value, NPC patients were divided into low expression group and high expression group. After radiotherapy, serum circTRAF3 expression level was significantly increased(P<0.05), and the expression increased gradually with the increase of radiotherapy times. The baseline serum circTRAF3 expression level in the resistance group was significantly higher than that in the sensitive group(P<0.05). Logistic regression analysis showed that baseline serum circTRAF3 level was an independent predictor of radiosensitivity in patients with locally advanced NPC(P<0.05). The area under the receiver operating characteristic curve of serum circTRAF3 for predicting the sensitivity of CCRT in patients with locally advanced NPC was 0.825(95%CI: 0.748-0.902). The postoperative progression-free survival, overall survival, distant metastasis-free survival and local progression-free survival of patients with locally advanced NPC in the high-expression group of serum circTRAF3 at baseline were significantly shorter than those in the low-expression group(P<0.05). Conclusion: circTRAF3 can be used as a good prognostic biomarker for locally advanced NPC and may affect radiotherapy efficacy and survival prognosis by modulating the radiosensitivity of patients.
