异常表达的PVT1作为诊断标志物预测重症肺炎合并呼吸衰竭患儿临床结局的研究

摘要:

目的:探讨lncRNA PVT1在重症肺炎合并呼吸衰竭患儿中的表达情况,进一步评估PVT1对疾病的诊断及预后价值。方法:采用实时荧光定量PCR技术对82例受试者进行血清PVT1表达水平的检测;采用Pearson相关系数法评估PVT1水平与miR-16-5p水平以及患儿临床病理指标的相关性;受试者工作特征(ROC)曲线用来评估PVT1、miR-16-5p以及二者联合对重症肺炎合并呼吸衰竭的诊断价值。利用Kaplan-Meier生存曲线和多因素Cox回归分析评估PVT1对重症肺炎合并呼吸衰竭的预后价值。荧光素酶报告基因实验用来确认PVT1与miR-16-5p的3'-非翻译区(3'-UTR)的相互作用。结果:与健康对照组相比,疾病组血清PVT1的表达水平出现显著上调趋势,而miR-16-5p的表达则明显降低;血清miR-16-5p的表达水平与PVT1的水平呈显著负相关性;ROC曲线表明PVT1、miR-16-5p及二者联合均显示出对疾病诊断的临床价值;生存分析表明PVT1高表达的患儿出现死亡事件的概率显著高于PVT1低表达患儿;荧光素酶报告基因实验显示PVT1直接靶向miR-16-5p。结论:高表达的PVT1是重症肺炎合并呼吸衰竭的良好的诊断标志物,并且与疾病最终的不良预后息息相关。

Objective: To investigate the expression of lncRNA PVT1 in pediatric patients with severe pneumonia complicated with respiratory failure, and to further evaluate the diagnostic and prognostic value of PVT1 for the disease. Methods: The expression levels of serum PVT1 in 82 subjects were detected by real-time quantitative PCR. Pearson correlation coefficient method was used to evaluate the correlation between PVT1 and miR-16-5p, as well as clinicopathological indicators of patients. Receiver operating characteristic(ROC) curves were used to assess the diagnostic value of PVT1, miR-16-5p and the combination of PVT1 and miR-16-5p in severe pneumonia complicated with respiratory failure. Kaplan-Meier survival curve and multivariate Cox regression analysis were performed to estimate the prognostic value of PVT1 in pediatric patients with severe pneumonia complicated with respiratory failure. The luciferase reporter gene assay was used to verify the interaction between PVT1 and miR-16-5p. Results: Compared with the healthy control group, the expression level of serum PVT1 in the patient group increased significantly, while the expression of miR-16-5p decreased. The level of miR-16-5p in serum was negatively correlated with the level of PVT1. ROC curve investigated that both PVT1 and miR-16-5p and their combination showed high diagnostic values in disease diagnosis. Survival analysis showed that the probability of death in pediatric patients with high level of PVT1 was higher than that in pediatric patients with low expression of PVT1. Luciferase reporter gene assay showed that PVT1 directly targeted with miR-16-5p. Conclusion: Highly expressed PVT1 is a good diagnostic marker for severe pneumonia complicated with respiratory failure and is associated with poor prognosis of the disease.
