CYP3A4基因多态性对羟考酮抑制腰椎手术后疼痛的影响

摘要:

目的: 观察细胞色素P450 3A4酶(CYP3A4)基因多态性对羟考酮抑制腰椎手术后疼痛的影响。方法: 静吸复合全身麻醉下行腰椎手术患者150例, ASAⅠ或Ⅱ级, 按照术前CYP3A4基因型检测结果分成3组: 野生型纯合子(CYP3A4*1/*1, n=80)标记为AA组, 突变杂合子(CYP3A4*1/*1G, n=60)标记为GA组, 突变纯合子(CYP3A4*1G/*1G, n=10)标记为GG组。所有患者均于手术完成前30 min静脉推注羟考酮5 mg, 同时予以静脉泵镇痛。于术后10 min(T0)、6 h(T1)、12 h(T2)和24 h(T3)进行疼痛强度数字评估量表(NRS)评分, 同时抽取静脉血2 ml测定血浆皮质醇(Cor), 记录术后24 h羟考酮的使用剂量、患者自控静脉镇痛(PCIA)按压次数、无效按压次数和不良反应(恶心、呕吐和呼吸抑制)的发生情况, 同时计算术后24 h内时间加权平均动脉压(TWA-MAP)和时间加权平均心率(TWA-HR)。结果: (1) 与AA组比较, GA和GG组患者在各时间点的疼痛强度NRS评分降低, 在T1、T2、T3时间点血浆Cor水平也降低(P < 0.05)。(2) 与AA组比较, GA和GG组PCIA按压次数和无效按压次数减少(P < 0.05), 羟考酮的总使用量也减少(P < 0.05), 恶心呕吐的发生率也明显降低(P < 0.05);但3组间呼吸抑制的发生率差异无统计学意义(P > 0.05)。(3) 与AA组比较, GA和GG组TWA-MAP和TWA-HR明显降低(P < 0.05)。结论: CYP3A4基因多态性是影响羟考酮镇痛效果的遗传因素。

Objective: To observe the effects of CYP3A4 gene polymorphism on inhibiting the postoperative pain of oxycodone in lumbar surgery.Methods: 150 patients undergoing selective lumbar surgery with general anesthesia were divided into three groups according to genotype: Wild-type homozygous (CYP3A4*1/*1, n=80) named AA group, Mutant heterozygous (CYP3A4*1/*1G, n=60) named GA group, Mutant homozygous (CYP3A4*1G/*1, n=10) named GG group. All patients received intravenous injection of 5 mg oxycodone 30 min before the end of the surgery, and continuous analgesia with intravenous analgesia pump. Numerical rating scales (NRS) were evaluated and venous blood was drawn to detect plasma cortisol at 10 min (T0), 6 h (T1), 12 h (T2), and 24 h (T3) after surgery. In addition, the total dose of oxycodone, the number of patient controlled intravenous analgesia(PCIA) compression and the ratio of adverse reactions (nausea, vomiting, and respiratory depression) were recorded, and the time-weighted mean arterial pressure (TWA-MAP) and time-weighted mean heart rate (TWA-HR) within 24 hours after surgery were calculated.Results: (1) Compared with the AA group, the NRS score in GA and AA groups decreased at all time points, and the level of Cor declined at T1, T2 and T3(P < 0.05). (2) Compared with the AA group, the number of PCIA compression and the total dose of oxycodone in GA and GG groups were all reduced, therefore the ratio of nausea and vomiting was declined accordingly, but there was no significant difference in incidence of respiratory depression among the three groups(P > 0.05). (3) Compared with the AA group, the TWA-MAP and TWA-HR in GA and AA groups decreased significantly(P < 0.05).Conclusion: CYP3A4 gene polymorphism is a genetic factor affecting the analgesic effect of oxycodone.
