血清TNF-α、IL-6、CRP水平对老年急性脑梗死患者介入治疗后睡眠障碍的预测价值

摘要:

目的: 探讨血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平对老年急性脑梗死(ACI)患者介入治疗后睡眠障碍发生风险的预测价值。方法: 收集本院2017年4月至2020年8月接受介入治疗且完成6个月随访的137例老年ACI患者的临床资料, 患者均接受血清TNF-α、IL-6、CRP水平检测。统计患者睡眠障碍情况并分组, 分析血清TNF-α、IL-6、CRP水平与老年ACI患者介入治疗后发生睡眠障碍的关系。结果: 137例老年ACI患者介入治疗后有51例发生睡眠障碍, 占37.23%;睡眠障碍组TNF-α、IL-6、CRP水平均高于睡眠良好组(P < 0.05); Logistic回归分析显示, 血清TNF-α、IL-6、CRP过表达可能是老年ACI患者介入治疗后发生睡眠障碍的风险因子(OR > 1, P < 0.05);绘制ROC曲线发现, 血清TNF-α、IL-6、CRP单一及联合预测老年ACI患者介入治疗后发生睡眠障碍风险的AUC均 > 0.80, 均有一定预测价值; Spearman相关分析显示, TNF-α、IL-6、CRP三者水平均呈正相关(r > 0, P < 0.05)。结论: 血清TNF-α、IL-6、CRP过表达与老年ACI患者介入治疗后发生睡眠障碍有关。

Objective: To analyze the value of serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), C-reactive protein (CRP) levels in predicting the risk of sleep disorder in elderly patients with acute cerebral infarction (ACI) after interventional therapy.Methods: The clinical data of 137 elderly ACI patients who received interventional treatment and completed 6 months of follow-up from April 2017 to August 2020 in our hospital were collected, serum TNF-α, IL-6 and CRP levels were detected. The sleep disorder patients were counted and grouped, the relationship between serum TNF-α, IL-6 and CRP levels and sleep disorders after interventional therapy in elderly ACI patients was analyzed.Results: In 137 elderly patients with ACI after interventional therapy, 51 patients had sleep disorder, accounting for 37.23%; the levels of serum TNF-α, IL-6 and CRP in sleep disorder group were higher than those in the sleep well group (P < 0.05); Logistic regression analysis showed that the overexpression of serum TNF-α, IL-6 and CRP alone and in combination in the elderly patients with ACI might be the risk factors of sleep disorder after interventional therapy (OR > 1, P < 0.05); the ROC curve was drawn, the AUC of serum TNF-α, IL-6, CRP alone and in combination in predicting the risk of sleep disorder in the elderly patients with ACI after interventional therapy were all >0.80, all had certain predictive value; Spearman correlation test showed that the levels of TNF-α, IL-6 and CRP were positively correlated(r > 0, P < 0.05).Conclusion: The overexpression of serum TNF-α, IL-6 and CRP in the elderly patients with ACI is related to sleep disorder after interventional therapy.
