目的：评估血小板（Plt）相关参数在急性心肌梗死（AMI）与不稳定型心绞痛（UA）鉴别诊断中的应用价值。方法：选择2017年2月至2020年6月来我院就诊的93例急性冠脉综合征患者作为研究对象，将其分为AMI组49例和UA组44例，比较两组患者的Plt计数、平均血小板体积（MPV）和血小板分布宽度（PDW），利用受试者工作特征（ROC）曲线分析Plt相关参数对AMI与UA的鉴别诊断效能。结果：AMI组患者的MPV和PDW明显高于UA组患者（P=0.041和P<0.001），MPV和PDW鉴别诊断的ROC曲线下面积（AUC）分别为0.623和0.768，PDW对AMI的诊断效能优于MPV。结论：PDW和MPV可作为急性冠脉综合征患者AMI与UA鉴别诊断的辅助指标。
