肝细胞癌组织中基质金属蛋白酶14的表达及其与预后的关系

摘要:

目的:探讨基质金属蛋白酶14(MMP14)在肝细胞癌(HCC)组织中的表达及其与患者预后的关系。方法:收集2008年6月至2013年5月本院手术切除的248例HCC癌组织标本及其配对的癌旁组织标本(距离癌组织边缘≥5 cm)。随机选取6例癌组织及其配对的癌旁组织,用Western blotting检测组织中MMP14蛋白相对表达量。采用免疫组织化学染色法检测所有组织中MMP14的表达水平,根据染色结果将HCC患者分为高表达组和低表达组。比较两组患者的临床病理特征和生存预后。结果:HCC癌组织中MMP14蛋白相对表达量为0.91±0.08,明显高于癌旁组织的0.24±0.08(P<0.05)。癌组织中MMP14高表达率为62.10%,明显高于癌旁组织的26.61%(P<0.001)。MMP14与HCC分化程度及TNM分期有关(P<0.05)。肿瘤低分化、高TNM分期和MMP14高表达是HCC患者总生存期和无瘤生存期的独立影响因素(P<0.05)。MMP14高表达患者的总体生存时间和无瘤生存时间明显低于低表达患者(P<0.05)。结论:HCC癌组织中MMP14表达水平明显高于癌旁组织。MMP14是HCC预后的独立影响因素,其表达水平越高,患者总生存期和无瘤生存期越短。

Objective: To investigate the expression of matrix metalloproteinase-14(MMP14) in hepatocellular carcinoma (HCC) and its relationship with prognosis. Methods: From June 2008 to May 2013, 248 cases of HCC tissues and their matched para cancerous tissue samples (distance from the edge of cancer tissue ≥ 5 cm) were resected in our hospital. Western blotting was used to detect the relative expression of MMP14. Immunohistochemical staining was used to detect the expression of MMP14 in all tissues. According to the staining results, HCC patients were divided into high expression group and low expression group. The clinicopathological features and survival prognosis of the two groups were compared. Results: The relative expression of MMP14 in HCC tissues was (0.91±0.08), which is significantly higher than that in adjacent tissues (0.24±0.08, P<0.05). The expression rate of MMP14 in cancer tissues was 62.10%, which was significantly higher than that in the adjacent tissues (26.61%,P<0.001). MMP14 was associated with HCC differentiation and TNM stage (P<0.05). Poor tumor differentiation, high TNM stage and high expression of MMP14 were independent influencing factors of overall survival and tumor free survival (P<0.05). The overall survival time and tumor free survival time of patients with high expression of MMP14 were significantly shorter than those with low expression of MMP14 (P<0.05). Conclusion: The expression of MMP14 in HCC tissues is significantly higher than that in the adjacent tissues. The higher the expression of MMP14, the shorter the overall survival and tumor free survival.
