外伤性肩袖损伤镜下修复和小切口治疗效果的对比分析

摘要:

目的:比较外伤后肩袖损伤镜下修复和小切口切开修复的临床疗效。方法:2012年3月至2016年3月因外伤导致的肩袖损伤在徐州市中心医院接受手术修复并获得定期随访的病人43例,其中接受关节镜镜下铆钉修复手术27例(镜下缝合组),小切口切开铆钉修复手术16例(小切口组),术后行相同的康复锻炼方案。比较两种手术方式的平均手术时间、平均出血量及手术疗效,手术疗效使用UCLA评分系统进行评价。结果:平均手术时间镜下缝合组稍长于小切口组,差异具有统计学意义(P<0.05);平均出血量镜下缝合组少于小切口组,差异具有统计学意义(P<0.05)。术后随访9~24个月,平均15个月,镜下缝合组优24例,良3例,无感染等并发症;小切口组优12例,良2例,差2例,1例迟发型化脓性感染,1例糖尿病患者急性感染,连续两次清创后愈合。两组平均UCLA评分差异无统计学意义(P>0.05)。结论:肩袖损伤镜下修复与小切口切开修复疗效相当,但并发症少于小切口切开修复。
