山西人群维生素D水平与2型糖尿病的相关性研究

摘要:

目的:探讨山西人群中维生素D水平与2型糖尿病(T2DM)之间的关系。方法:选取196例T2DM患者为T2DM组,同期选取99例健康体检者作为对照组。检测血清25羟维生素D3[25(OH)D3]、糖代谢指标[空腹血糖(FPG)、胰岛素(FINS)、糖化血红蛋白(HbA1c)]、C肽以及脂代谢指标[甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)]水平。结果:T2DM组血清25(OH)D3水平明显低于对照组(P<0.01);维生素D水平变化具有明显季节特性(P<0.01);相关分析显示维生素D水平与FPG、HbA1C、C肽之间呈负相关,与FINS无相关性;调整混杂因子后Logistic回归分析显示,与维生素D水平正常[25(OH)D3≥30 ng·ml-1]的人群相比,维生素D水平缺乏和不足[25(OH)D3<30 ng·ml-1]的人群患T2DM的风险增加(OR=2.46,95%CI为1.18~5.16)。结论:山西人群T2DM患者中普遍存在维生素D缺乏的现象,并且具有明显的季节特性。

Objective: To explore the status of serum 25-hydroxyvitamin D[25(OH)D3] and the relationship with type 2 diabetes mellitus(T2DM) in Shanxi. Methods: A total of 196 patients with T2DM were enrolled in this study. In the same period, 99 healthy people were selected as the control group. Serum 25(OH)D3 levels and glucose, lipid metabolism indicators were measured. Results: Serum 25(OH)D3 concentration in patients with T2DM was significantly lower than that in the control group (P<0.01). Serum 25(OH) D3 levels were associated with seasonal variation in both healthy and T2DM population (P<0.01). Correlation analysis showed that there was a negative correlation between 25(OH)D3 and FPG, HbA1C, C peptide, and no correlation with FINS. The risk of having T2DM increased significantly when vitamin D levels decreased from ≥ 30 ng·ml-1 to <30 ng·ml-1 after adjustment for confounders (OR=2.46, 95%CI: 1.18-5.16). Conclusion: Vitamin D deficiency is prevalent in Shanxi population and has obvious seasonal characteristics.
