血清骨形成标志物在学龄前儿童体格发育评估中的应用

摘要:

目的:探讨分析血清骨形成标志物在学龄前儿童体格发育评估中的应用。方法:选择我院就诊的85例特发性身材矮小学龄前儿童作为观察组,选择同期健康体检的健康学龄前儿童80例作为对照组,对两组儿童进行体格检查,并检测儿童血清骨碱性磷酸酶(AKP)、骨钙蛋白(OC)以及I型胶原羧基端前肽(PICP)水平。结果:观察组儿童身高、体重均明显低于对照组(P<0.05);观察组儿童血清AKP、OC以及PICP水平均显著低于对照组(P<0.05);儿童身高与血清AKP、OC以及PICP水平呈显著正相关关系(P<0.05),儿童体重与AKP呈显著正相关关系(P<0.05)。采用ROC曲线分析,儿童血清AKP对特发性身材矮小检出曲线下面积为0.961,OC对特发性身材矮小检出曲线下面积为0.770,PICP对特发性身材矮小检出曲线下面积为0.778。结论:血清骨形成标志物对儿童体格发育评估具有着重要价值,可反映儿童骨骼发育情况,有助于早期检出生长迟缓儿童,值得临床推广运用。

Objective:To explore the application of markers of serum bone formation in the assessment of physical development of preschool children. Methods:Eighty-five cases of idiopathic short stature primary school children in our hospital were selected as observation group, and 80 healthy preschool children were selected as control group for the same period of health examination.The two groups of children were examined and the levels of serum bone alkaline phosphatase (AKP), osteocalcin (OC) and type I collagen carboxyl terminated propeptide (PICP) were detected in children. Results:The height and weight of the children in the observation group were significantly lower than those in the control group, the difference was statistically significant (P<0.05). Serum AKP, OC and PICP levels in the observation group were significantly lower than those in the control group (P<0.05). There was a significant positive correlation between children's height and serum AKP, OC and PICP levels (P<0.05), and there was a significant positive correlation between body weight and AKP (P<0.05). As of the ROC curve analysis, the area of children's serum AKP against idiopathic short stature was 0.961, the area under the detection curve of OC for idiopathic stature was 0.770, and the area under PICP for the detection of idiopathic short stature was 0.778. Conclusion:The serum bone formation marker is of great value in assessing the physical development of children. It can reflect the development of the bone in children, and it is helpful for the early detection of growth retarded children. It is worth popularizing in clinical practice.
