基于声触诊组织成像和定量技术鉴别诊断乳腺实性肿瘤

摘要:

目的:采用声辐射力组织成像和定量技术鉴别诊断乳腺实性肿瘤。方法:对甲乳外科住院的乳腺肿瘤患者行常规二维超声检查,对BI-RADS分类在3类及以上且符合入组条件的106例肿瘤患者行声辐射力剪切波弹性成像。测量肿块内部硬度最大的9点剪切波速度,得出良恶性肿瘤剪切波速度的最佳界值。结果:106例乳腺肿瘤中,39例乳腺恶性肿瘤内部最大9点剪切波速度显著高于67例良性肿瘤内部最大9点剪切波速度[(6.00±0.61)m·s-1 vs(3.66±0.32)m·s-1t=8.870,P=0.000],以剪切波速度4.74 m·s-1为界值,诊断乳腺实性肿瘤的敏感性为82.1%(32/39),特异性为91.0%(61/67)。结论:乳腺实性恶性肿瘤内部最大剪切波速度明显高于乳腺良性肿瘤。声辐射力组织成像和定量技术可提高诊断乳腺肿瘤的特异性。

Objective:To investigate the diagnostic value of virtual touch tissue imaging and quantification in the differential diagnosis between benign and malignant solid breast masses. Methods:A total of 106 breast masses were examined by conventional ultrasound and the virtual touch tissue imaging quantification technique. The average shear wave velocity (SWV) value of the 9 hardest points of the lesions were obtained, and the optimal cut-off value of breast lesions was measured. Results:In 106 breast lesions, the average SWV value of malignant breast lesions was higher than that of benign ones[(6.00±0.61) m·s-1 vs (3.66±0.32) m·s-1, t=8.870, P=0.000]. The sensitivity and specificity for identifying breast lesions on a SWV cut-off value of 4.74 m·s-1 were 82.1%(32/39) and 91.0%(61/67),respectively. Conclusion:The virtual touch tissue imaging and quantification can improve the capability in differential diagnosis of breast lesions.
