脑卒中患者Hcy、GFAP、BDNF和hs-CRP水平与血管性认知障碍相关性研究

摘要:

目的:探讨脑卒中患者同型半胱氨酸(homocysteine,Hcy)、胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)、脑源性神经营养因子(brain derived neurotrophic factor,BDNF)和超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)水平与血管性认知障碍的相关性。方法:选取2014年5月至2017年5月本院收治120例脑卒中患者为观察组,分为无痴呆型血管性认知障碍(vascular cognitive impairment no dementia,VCIND)组、血管性痴呆(vascular dementia,VD)组和混合性痴呆(mixed dementia,MD)组,每组40例;另选取40例健康体检者为对照组。收集研究对象的血液并检测Hcy、GFAP、BDNF和hs-CRP含量。结果:各型脑卒中患者血管性认知障碍与血清Hcy、GFAP和hs-CRP呈显著正相关;随着血管性认知障碍程度加重,血清Hcy、GFAP和hs-CRP含量显著升高,差异具有统计学意义(P<0.05)。血管性认知障碍与血清BDNF蛋白存在显著的负相关关系;随着血管性认知障碍程度加重,血清BDNF含量水平越低,差异具有统计学意义(P<0.05)。结论:脑卒中患者体内Hcy、GFAP、BDNF和hs-CRP水平改变明显,且与血管性认知障碍的3种主要类型呈显著的相关性。

Objective:To investigate the effects of Homocysteine (Hcy), glial fibrillary acidic protein (GFAP), brain derived neurotrophic factor (BDNF) and high sensitivity C-reactive protein (Hs-CRP) levels and vascular cognitive impairment. Methods:From May 2014 to May 2017, 120 patients with stroke were enrolled in this study. Forty patients with vascular cognitive impairment (VCIND), 40 cases of vascular dementia (VD) and 40 patients with mixed dementia (MD) together with 40 healathy controls were enrolled in this study. Patients' blood were collected and the contents of Hcy, GFAP, BDNF and hs-CRP were measured. Result:The classification of vascular cognitive impairment in patients with stroke was significantly related to the serum levels of Hcy, GFAP and hs-CRP protein in patients with dementia vascular cognitive impairment (VCIND), vascular dementia (VD) and mixed dementia (MD). There were a significant positive correlation between the levels of Hcy, GFAP and hs-CRP in patients with vascular cognitive impairment, the differences were statistically significant (P<0.05).There was a significant negative correlation between serum BDNF and serum BDNF. With the aggravation of vascular cognitive impairment, the level of serum BDNF was lower, the difference was statistically significant (P<0.05). Conclusion:The levels of homocysteine(Hcy), glial fibrillary acidic protein (GFAP), brain-derived neurotrophic factor BDNF and hs-CRP in stroke patients are significantly changed. And the three major types of vascular cognitive impairment is significantly correlated.
