Objective:To study the effect of Wiltse approach and posterior median approach TLIF of lumbar disc herniation patients and its influence of creatine kinase (CK), digital classification (NRS) and disability index (ODI) score.Methods:82 cases of lumbar disc herniation were selected from February 2015 to February 2016 in our hospital, who were randomly divided into observation group and control group, with 41 cases in each group. All patients were treated with TLIF procedure, the control group was treated via posterior approach, the observation group was treated via Wiltse approach. JOA scores of two groups of the patients before and after surgery, CK level, NRS score, ODI score and visual analogue score (VAS) changes were compared. Results:JOA scores were significantly higher than those of before operation, and JOA score of the observation group after surgery was significantly higher than that of the control group, the differences were statistically significant (P < 0.05). The level of CK and ODI scores of Postoperative patients in the observation group were (163.5±23.8) U·L-1, (23.4±2.6), which were significantly lower than those the control group (407.5±38.6) U·L-1, (27.9±4.9), the differences were statistically significant (P<0.05). Low back pain, lower limb pain and numbness of the NRS scores in postoperative observation group were significantly less than those of the control group, the differences were statistically significant (all P<0.05). VAS score of postoperative observation group and the control group were respectively (1.5±0.4), (2.1±0.7), significantly lower than that before surgery, while it was significantly lower in the observation group than that in the control group, the differences were statistically significant (P<0.05). Conclusion:The curative effect of Wiltse approach TLIF in the treatment of lumbar disc herniation is significant in improving the CK level of patients, relieving the clinical symptoms, and promoting the spinal function recovery of patients and reducing the pain. |
