Objective: To investigate the postoperative analgesic effect of dexmedetomidine combined with pentazocine in elderly patients with total hip replacement and the influence on the levels of inflammatory factors,fibrinogen (Fib) and D-dimer. Methods: Ninety-six elderly patients treated by total hip replacement were enrolled in this study. They were randomly divided into the control group (n=48) and the observation group (n=48) according to the random number table method. The control group was given pentazocine, while the observation group was given dexmedetomidine on the bases to ease pain. Visual analogue scale (VAS), levels of serum tumor necrosis factorα(TNF-α), interleukin 6 (IL-6), interleukin 8 (IL-8), Fib, D-dimer, and adverse reactions were compared between the two groups after operation.Results: Compared with the control group, the VAS of the observation group were statistically lower at 6, 12 and 24 h after operation[(2.35±0.94)vs.(3.96±1.05)points,(1.66±0.71)vs.(2.87±0.82)points,(1.32±0.52)vs.(2.15±0.63)points, respectively] and all P<0.05. Twenty-four hours after operation, the observation group had lower levels of TNF-α,IL-6, IL-8[(2.02±0.67)vs.(3.95±0.96)pg·ml-1,(32.24±4.76)vs.(40.16±5.25)pg·ml-1,(20.37±4.26)vs.(30.29±5.71)pg·ml-1, respectively] and lower levels of Fib, D-dimer[(3.9±0.3)vs.(4.2±0.4)g·L-1,(224.7±18.5)vs.(251.3±20.2)ng·ml-1, respectively] than those in the control group (all P<0.05). After operation, the incidences of adverse reactions in the control group and the observation group were 8.3% and 14.6% respectively with no statistical difference (P>0.05) and there was no cases of severe adverse reactions. Conclusion: Dexmedetomidine combined with pentazocine has good postoperative analgesic effect in elderly patients with total hip replacement.And it can effectively ease postoperative pain, alleviate inflammatory reaction and improve blood hypercoagulability with minor adverse reactions. |
