哈萨克族原发性高血压患者血清胆红素及血脂水平的分析

摘要:

目的:研究哈萨克族原发性高血压(EH)患者血清胆红素与血压、血脂的关系,并探讨胆红素水平的性别差异。方法:选取2014年11月至2015年4月接受流行病学调查的新疆哈萨克族人群267人作为研究对象,根据血压分为EH组(128例)和血压正常组(139例),对比分析各组总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C),计算体重指数(BMI);另外根据性别将两组人群各分为两个亚组,对比分析各组血清胆红素水平。结果:与正常组比较,EH组TBIL、DBIL均较低(P<0.05),年龄、BMI、TC、LDL-C均较高(P<0.05)。各亚组组间比较,EH组女性较正常组女性TBIL、DBIL、IBIL均降低(P<0.05),较EH组男性,TBIL、DBIL均降低(P<0.05)。相关分析显示TBIL、DBIL与LDL-C呈负相关(r分别为-0.176、-0.233,P均<0.05),DBIL与TC呈负相关(r=-0.192,P<0.05)。结论:新疆哈萨克族EH患者的血清胆红素水平降低,BMI、TC、LDL-C升高;血清总胆红素及直接胆红素与血压有一定的相关性,并可能是哈萨克族女性患EH的危险因素;血清TBIL、DBIL可能是通过影响血脂代谢而影响血压的。

Objective: To investigate the relationship between serum bilirubin levels and blood pressure, lipids in the Kazak patients with essential hypertension(EH), and explore the gender differences of serum bilirubin levels. Methods: 128 cases of EH and 139 cases of health controls were collected from Xinjiang Kazak population through epidemiological investigation from November 2014 to April 2015,the serum total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL),triglycerides(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C) were measured,and body mass index(BMI) were calculated; and the serum bilirubin levels were analyzed in male and women groups. Results: The TBIL, DBIL in EH group were lower than those in control group(P<0.05), but the age,BMI,TC,LDL-C were higher(P<0.05). The TBIL, DBIL, IBIL in women EH group were lower than those in women control and the male in EH group(P<0.05).Pearson relation analysis showed that TBIL and DBIL were negatively correlated with LDL-C(r values were-0.176,-0.233, respectively,P<0.05, overall), and DBIL also negatively correlated with TC(r=-0.192, P<0.05). Conclusion: The serum TBIL and DBIL have a certain correlation with blood pressure, and may become a risk factors for women of the Kazak patients with EH;Serum TBIL and DBIL may affect the blood pressure through blood lipid metabolism.
