超极速超声成像脉搏波技术在早期颈动脉粥样硬化风险动态评估中的应用价值

摘要:

目的:利用超极速超声成像脉搏波(ufPWV)技术早期发现颈动脉粥样硬化(As),动态评价血管弹性变化及早期动脉粥样硬化进展程度。方法:检测对照组正常人59例(118条颈动脉)和As风险组患者75例(149条颈动脉),测量颈动脉内膜中层厚度(IMT),再利用ufPWV技术检测收缩早期脉搏波速度(BS)和收缩末期脉搏波速度(ES)。两组均进一步分为A组(IMT 0.049cm及以下)、B组(IMT 0.050~0.069 cm)、C组(IMT 0.070cm及以上)3个亚组。对数据进行统计分析。结果:As风险组与对照组比较,IMT、BS及ES差异均有统计学意义(P<0.001)。两组的A组ES比较,差异有统计学意义(P<0.001)。两组IMT在0.040~0.054cm时,ES与IMT呈线性相关(r=0.585,P<0.001);IMT在0.055cm及以上时,ES达到可能存在的阈值而保持相对稳定。结论:ufPWV技术可发现IMT尚未明显增厚的早期颈As,并可通过ES的变化动态评估早期As的进展程度。

Objective:To conduct an early detection of carotid artery atherosclerosis (As) by the use of technology of ultra-fast ultrasonic imaging pulse wave(ufPWV),conduct a dynamic evaluation of the change in vascular elasticity and progression level of early-stage atherosclerosis.Methods:Carotid intima media thickness(IMT) of 59 healthy subject(118 carotid arteries) in the control group and 75 patients (149 carotid arteries)in the risk group were detected,then the ufPWV technology was used to detect the beginning-systole pulse wave speed (BS) and end-systole pulse wave speed(ES).The two groups were divided into three subgroups respectively:group A(IMT 0.049 cm or less),group B(IMT 0.050-0.069cm),group C(IMT 0.070cm or more).The data was statistically analyzed.Results:The differences in IMT,BS and ES between the carotid As risk group and control group were all statistically significant (P<0.001).The difference in subgroup A between both groups was statistically significant (P<0.001). When IMT was 0.040-0.054cm,ES was linearly correlated with IMT(r=0.585,P<0.001);when IMT was 0.005cm or more,ES reached a possible threshold and then remained stable relatively.Conclusion:ufPWV technology can be used to detect the early-stage carotid As whose IMT hasn't yet significantly been thickened,and dynamically evaluate the progression level of early-stage carotid As through the change in ES.
