脑干梗死后低频振幅静息态fMRI的相关研究

摘要:

目的:采用静息态功能磁共振成像(fMRI)技术,探讨脑干梗死后脑功能网络变化及其可能的神经机制.方法:收集临床确诊的19例桥脑梗死患者的临床资料,同时选取年龄和性别相匹配的健康志愿者19名作为正常对照组.采集所有被试者静息态fMRI数据.运用DPARSFA软件对数据进行后处理,运用低频振幅(ALFF)方法观察脑干梗死脑区活动情况,分析桥脑梗死患者与正常对照组相比ALFF的相关改变,运用Rest1.8软件进行双样本t检验并对结果进行统计学分析.结果:脑干梗死组与正常对照组相比ALFF显著降低区域包括扣带回、两侧顶下小叶、两侧楔前叶、小脑及右侧海马,ALFF显著增高区域包括两侧额上回、中央前回(背侧额前叶)、左侧丘脑及尾状核.结论:脑干梗死后脑功能网络活动存在异常,背侧额前叶及丘脑活动升高,提示脑干梗死后脑功能网络的重构或代偿.

Objective: To observe the changes and to explore its possible neural mechanisms in brain networks after brainstem infarction by using resting-state functional MRI(fMRI) techniques. Methods: Nineteen patients diagnosed with isolated pontine infarction(IPI) by clinic were selected as brainstem infarction group, while nineteen healthy volunteers matched the age and gender were selected as the normal control group. Resting-state fMRI data was obtained using Philips Achieva 3.0 Tesla MRI scanner. DPARSFA software was used for data processing, Cerebra functional connectivity was observed by using amplitude of low-frequency fluctuation(ALFF)and the differences between the two groups were compared by using two-sample t-test analysis. Results: Compared with normal control group, significant decreased areas by using ALFF in brainstem infarction group include cingulate gyrus, inferior parietal lobule on both sides, the front side precuneus, and cerebellum, right hippocampus; significantly increased areas include the amount of both sides of the back/medial frontal gyrus(dorsal prefrontal lobe), left thalamus and caudate nucleus. Conclusion: Abnormal activities of brain network after brainstem infarction are observed. Increased activities in dorsal frontal anterior lobe and thalamus maybe prompt reconstruction or compensatory function of the network after brainstem infarction.
