肿瘤血管新生与肾癌进展及患者预后的关系

摘要：

目的:研究肿瘤微血管密度(IMD)与肾癌进展及肾癌根治术后患者预后的关系.方法:采用免疫组织化学染色法对76例肾癌根治术的手术切除标本进行IMD定量研究,观察IMD与肾癌病理分级、临床分期及转移行为的关系.同时结合病例随访资料研究IMD对根治术后患者预后判断的价值.结果:随临床分期增加,IMD值增加(P<0.01);不同病理分级患者IMD值无显著性差异(P>0.05);IMD高者术后生存时间短;逐步回归分析表明,IMD与肾癌根治术后患者的预后关系密切.结论:IMD与肾癌的恶化、进展有关,并对肾癌根治术后患者预后的判断具有重要意义.

Objective To study the relationship of intratumor microvessel density(IMD) with progression and prognosis of renal cell carcinoma.Methods IMD was determined by immunohistochemical staining in renal cell carcinoma with anti-human factor Ⅷ-related antigen antibody.The relationship of IMD with Robson stage,clinical grade,metastasis and prognosis was studied.Results IMD levels increased with Robson stage,the IMD with different stage disease was not significantly different(P>0.05),IMD was higher in tumors with metastasis(P<0.01)and these renal cell carcinoma patients had a shorter survival time comparing to low IMD level group,with a very poor prognosis.Multivariate analysis(step wise regression)indicated that IMD was the most important prognostic factor in renal cell carcinoma patients after radical nephrectomy.Conclusion IMD is related to the progression and prognosis of renal cell carcinoma and has an important role in prognostic judgement of postoperative patients.
