Objective  The purpose of the investigation was to study valvular regurgitation,morphological changes and left ventricular function in patients with left ventricular enlargement.Methods  Thirty  five cases of dilated cardiomyopathy(DCM),20 of ischemic cardiomyopathy(ICM) and 20 of congestive heart failure were studied.2  dimensional echocardiography combined with M  mode echocardiography were used to evaluate the diameter of all four cardiac chambers,the pulsed Doppler recording and software were used to evaluate the left ventricular function,the color Doppler flow imaging was used to evaluate the valvular regurgitation.Results  All the patients with left ventricular enlargement had multivalvular regurgitation.The highest incidence of multivalvular regurgitation was found in the DCM group,which is mainly complicated with mitral regurgitation(91%).The ICM group was mostly complicated with mitral regurgitation(75%) and aortic regurgitation(70%).The multivalvular regurgitation was mildest in congestive heart failure group.The diameter of left atrium and left ventricle in three groups were significantly larger than that of the normal control group(  P  <0.001),but there was no significant difference between valvular regurgitation group and non  valvular regurgitation group.The systolic function was significantly changed in DCM group(70%),whereas the diastolic function was obviously deteriorated in ICM group(75%).Conclusion  The half quantification of valvular regurgitation combined with left ventricular functional evaluation can provide helpful clue for the diagnosis of left ventricular enlargement.
