异丙酚和咪唑安定在硬膜外麻醉行同种异体肾移植术中的应用

摘要：

目的:观察异丙酚、咪唑安定在硬膜外麻醉行同种异体肾移植术中的应用效果.方法:选择硬膜外麻醉效果欠佳的肾移植手术病人31例,随机分为咪唑安定组(Ⅰ组)、咪唑安定和异丙酚复合组(Ⅱ组)、异丙酚组(Ⅲ组),分别采用微量注射泵静脉注射辅助麻醉.记录病人术中血压、脉搏、血氧饱和度、见尿时间及清醒时间.结果:全部病人安静入睡,高血压控制满意,Ⅱ、Ⅲ组病人循环稳定,肾动脉开放后血压回升迅速,见尿快,极少使用缩血管药.结论:复合使用异丙酚和咪唑安定或单用异丙酚辅助硬膜外麻醉行肾移植术效果佳,有利于保护肾功能.

Objective To investigate the effects of propofol and midazolam on epidural anesthesia during kidney transplantation. Method Thirty-one American society of anesthesiologists Ⅱ～Ⅲ adult patients who needed asistant anesthesia were randomly devided into three groups,i.e.group Ⅰ:midazolam only;group Ⅱ:combination of propofol and midazolam;group Ⅲ:propofol only.During the operations the scores of SBP,DBP,PR,SaO 2,time of regaining consciousness and dose of vasoconstrictor drug were recorded for statistical analysis. Results All the patients slept quietly and the hemodynamics kept stable during anesthesia.Those patients of groupⅡand Ⅲ woke up more quickly and needed much more less vasoconstrictor drug than those of group Ⅰ. Conclusion Both propofol and its combination with midazolam had a good effect on epidural anesthesia in kidney transplantation.
