门控SPECT显像法测定心肌缺血范围和收缩功能的关系

摘要：

目的:研究99mTc-MIRI门控心肌显像的心肌缺血和心腔容积变化关系,了解门控心肌显像的心功能参数变化是否反映心肌缺血范围.方法:利用99mTc-MIBI作为显像剂,采用门控SPECT技术,一次显像得到心肌缺血范围、心室平均容积、舒张末期和收缩末期心室容积等心功能参数,作统计分析,观察不同缺血范围组间心室功能参数差异,以及心肌缺血范围是否和心室容量的增大呈正相关.结果:不同缺血范围的观察对象间单位体表面积心室容量存在差异性,缺血范围和心室扩大成正相关.结论:在共行显像条件下,门控心肌灌注显像不但能显示心肌缺血范围,其心室容量的变化也能反映心脏缺血导致心功能受损的严重程度.

Objective To investigate whether the heart function variation can reflect the myocardial ischemia extent.Methods The gated SPECT concurrent myocardial perfusion imaging was performed after injection of the 99mTc-MIBI.The extent of the myocardial ischemia and the heart function reference,such as volumes of end systolic,end diastolic and the average were statistically analyzed.The difference of ventricular functions among the myocardial ischemia extent was compared.The positive correlation was also evaluated between the extent of ischemia and the ventricular dilation.Results The difference of the ventricular volume in each heart motion phase existed in the myocardial ischemia objectives,and in different extent of ischemia group.A positive correlation existed between the ischemia extent and ventricular dilation.Conclusion The concurrent gated SPECT imaging,reveals extent of the myocardial ischemia,as well as the ventricular dilation,an important reference for the evaluation of the heart function.
