胆囊癌的手术治疗体会(附66例临床分析)

摘要：

目的:总结我院近10年来手术治疗的66例胆囊癌的治疗情况,探讨如何提高胆囊癌的手术治疗效果.方法:对66例胆囊癌的手术治疗病例作一回顾性分析,根据病情将手术分为意外胆囊癌、肝脏型、胆道型、肝胆型和其他型5种类型.结果:本组共行根治性切除术18例,包括胆囊、肝脏楔型切除术9例,半肝切除术/肝三叶切除术6例,胰十二指肠切除术3例,均进行了相应淋巴结清扫.活检包括单纯活检、活检加胆囊造瘘或胆管T管引流术,姑息性切除包括单纯胆囊切除、胆囊切除加胆管T管引流术或胆肠吻合术.结论:对胆囊癌患者根据肿瘤累及范围,可以采取规范的根治术、积极的扩大根治术和及时的再手术.

Objective To investigate how to improve the surgical therapeutic strategy for gallbladder cancer.Methods The surgical treatment in 66 cases of gallbladder cancer were analyzed retrospectively.According to the surgical procedure,the operation were divided into 5 types:unexpected gallbladder carcinoma,liver involved,bile duct involved,liver and bile duct involved,and the others.Results There are 18 cases curative resection in this study,including 9 cases of cholecystectomy followed by liver wedge resection,6 cases of hemihepatectomy/trisegmentectomy,3 cases of duodenopancreatectomy.All cases above were followed by lymphoid node resection.Conclusion According to the tumor involvement,formal radical,extended radical resection and re-operation should be considered in this fearful malignancy.
