原位大鼠肾脏移植模型技术的改进

摘要:

目的:为提高大鼠肾移植模型中供体肾脏的利用率,对供肾切取、大鼠输尿管吻合方法进行改进,建立从同一供体切取双侧肾脏分别进行原位肾脏移植的方法.方法:分别切取供体的左、右侧肾脏,经肾脏动脉进行灌注;供肾的肾动脉、静脉分别与受体左侧的肾脏动、静脉血管端端吻合,并应用暂时性内支架辅助的方法进行供肾输尿管与受体输尿管上段直接吻合,同时切除受体右侧肾脏.比较左、右侧供肾受体成活率及术后并发症,并通过病理切片评价移植物的组织学变化.结果:从30个供体大鼠切取52只肾脏,分别施行左、右侧供肾肾脏移植28、24例.与常规单侧供肾相比节约大鼠42%.左侧供肾组5d动物存活率89.3%(25/28),右侧供肾组5d动物存活率91.6%(22/24),采用左右侧供肾对受体存活率、移植物的组织学改变无明显影响.结论:这种方法能节省实验时间,减少供体的数量,符合动物实验的经济和伦理学要求.

Objective Kidney transplantation in rats is a widely used model in transplantation immunology.With the conventional method,only one kidney is harvested from the donor rat.For reducing the number of experimental animals used,we modified a technique for harvesting and transplanting two kidneys from one donor rat with microsugical technique.Methods After infusion of kidneys with HCA(hypertonic citrate adenine solution) kidney preservation solution,both kidneys were harvested one after another.The left donor kidney was transplanted orthotopically,and the right donor kidney was rotated and transplanted to the recipient left side.The end-to-end anastomosis of the renal artery and the renal vein was done.Ureter end to end anastomosis was performed using 6 interrupted full-thickness sutures by means of an 180° biangulation technique,introducing a 7/0 prolene guide within the lumen to allow easier placement of the anterior sutures.The right kidney of repicient was resected simultaneously.Recipients survival rate and operational complications were evaluated,graft morphology was assessed on paraffin sections.Results 52 kidneys were harvested from 30 donor rats,No differences were observed in graft survival rates and histology between orthotopically transplanted left kidneys(n=28) and transplanted right kidneys(n=24).We saved 22(42%) donor rats compared with conventional methods,resulting a clear reduction in the number of animals needed.Conclusion From an ethical as well as economical point of view,we have developed a technique which is simple,time-saving and can reduce the number of donor rats.
