组织多普勒成像测量Tei指数评价右心功能不全

摘要：

目的:探讨Tei 指数在评价右心功能不全中的临床价值.方法:右心功能不全患者组32例,正常对照组30例.超声心动图(TDI)条件下,于心尖四腔观、右心室流入道长轴观分别获得右室游离壁、室间隔、前壁和后壁近三尖瓣瓣环各位点组织多普勒频谱,测量每一位点舒张晚期Am波终止至下一舒张早期Em波起始的间期(a)和收缩期Sm波的间期(b),通过(a-b)/b计算4个位点Tei指数,取其均值t-Tei.在心尖四腔观用Simpson法测量右室射血分数(RVEF).结果:患者组较正常对照组t-Tei 指数明显延长(0.69±0.41 vs 0.28±0.16,P<0.01 );同一组别不同室壁间的Tei 指数无明显差异;患者组t-Tei测值与二维超声心动图Simpson 法计算的右室射血分数呈现很好相关性(r=0.850,P<0.01).结论:Tei 指数可综合评价右心功能不全右室整体收缩舒张功能.

Objective To estimate the value of Tei index of Doppler imaging(TDI)in the evaluation of right ventricular function.Methods The subjects included 32 patients with right heart failure and 30 healthy adults,TDI velocity profiles of tricuspid annulus at right ventricular free,septum,anterior and posterior walls were acquired at the apical four-chamber view and right ventricular inflow tract long-axis view.The time interval from the end-diaotolic Am wave to the early diastolic Em wave(a)and the duration of systolic phase Sm wave(b)were measured and Tei index was calculated from the equation:Tei index=(a-b)/b.The average of the four sites were calculated(t-Tei).The right ventricular ejection fraction(RVEF)was measured with Simpson method at apical four-chamber view.Results The t-Tei index in right heart failure group was prolonged than that of the normal group(0.69±0.41 vs 0.28±0.16,P<0.01).There was no significant differences among the Tei values of different sites,which was closely related to the RVEF in right heart failure group(r=0.850,P<0.01).Conclusion Tei index is valuable for estimating systolic and diastolic functions of right ventricular in patients with right heart failure.
