全胃切除术及消化道重建的发展与现状

摘要：

胃癌是世界上常见的恶性肿瘤。全胃切除术是目前根治胃癌的主要手段，术后的消化道重建更是对患者预后有着重要的影响。本文中作者回顾了近年来全胃切除术及消化道重建方式的相关研究。目前全胃切除术后的重建方式除Roux-en-Y食管空肠吻合术外，主要还有各种贮袋代胃和保留十二指肠路径的术式。已进行的多数研究表明，使用贮袋代胃和保留十二指肠路径相结合的方式往往意味着更好的体重恢复、更少的并发症和更高的生活质量，但是由于各研究样本量较少且组间无明显可比性，该结论尚需更加严谨的循证医学证据支持。而传统Roux-en-Y重建术，由于其简单有效仍被广泛应用。
