目的:通过对V级肝损伤治疗的回顾分析,探讨肝脏损伤的治疗方法。方法:对1例V级肝损伤患者的临床资料进行回顾性分析。结果:患者因"上腹部车祸伤1 h"入我院急诊,彩超示肝内血肿。因患者血流动力学稳定,予以非手术治疗。治疗后24 h后患者腹痛加剧,腹部彩超示腹腔大量游离液,遂行急诊手术。术中发现肝后下腔静脉、肝右静脉及肝中静脉破裂。以左手拇指及中指阻断下腔静脉破裂处的远、近端,行肝中静脉结扎、肝右静脉及下腔静脉修补术。结论:指压阻断下腔静脉止血效果满意,可保证下腔静脉及肝右静脉缝合术的顺利进行,术后患者恢复满意。
