经膀胱造瘘口采用气压弹道超声碎石清石系统治疗膀胱铸型结石(附一例报告)

摘要：

目的:探讨经耻骨上膀胱造瘘口气压弹道超声碎石清石系统治疗永久性膀胱造瘘合并膀胱铸型结石患者的临床疗效和意义。方法:采用连续硬膜外麻醉,经永久性膀胱造瘘口入镜,对1例需永久性膀胱造瘘合并膀胱铸型结石的患者行超声气压弹道碎石术。结果:该患者通过永久性膀胱造瘘口进镜碎石成功,碎石时间158 min。术中、术后无膀胱痉挛、大出血、膀胱穿孔、电解质紊乱等并发症。经结石成分红外光谱分析提示为感染性结石。结论:经耻骨上膀胱造瘘口行肾镜EMS气压弹道联合超声碎石清石术是治疗永久性膀胱造瘘合并膀胱结石的安全、有效的方法。

Objective: To discuss the clinical efficacy and significance of suprapubic bladder fistulization orifice barometric pressure trajectory ultrasound lithotripsy Qing stone system to treat permanent bladder fistula complicated with bladder cast stones. Methods: The permanent bladder fistulization orifice into the mirror, mold ultrasound pneumatic lithotripsy was done on one patient with requiring permanent cystostomy combined with bladder stones. Continuous epidural anesthesia was taken. Results: The lithotripsy time of the patient through permanent bladder fistulization orifice into the mirror was 158 minutes. During and after the operation, there was no bladder spasm, bleeding, perforation of the urinary bladder, electrolyte disorders and other complications. The composition of calculi infrared spectroscopic analysis of tips were the infective calculus. Conclusion: Suprapubic bladder stoma kidney mirror EMS combined pneumatic and ultrasound lithotripsy in the treatment of permanent cystostomy with big bladder stones is a safe, effective method.
