纯化内耳组织抗原免疫致豚鼠自身免疫性梅尼埃病的实验研究

摘要:

目的: 探讨4种纯化的内耳抗原免疫致梅尼埃病样内耳病变的情况,从而了解各抗原的致病性,并明确其在内耳的分布情况。方法: 70只豚鼠随机分成A、B、C、D、E 5组(A组10只,余4组各15只),以豚鼠内耳组织抗原的4种亚组分(相对分子质量依次为28 000、42 000、58 000和68 000)作为抗原,依次免疫B、C、D和E组豚鼠,观察听觉功能、前庭功能、血清特异性抗体水平和内耳形态学的改变,并应用免疫组化方法确定其在耳蜗的分布和表达。结果: 免疫前后A(对照)、B组平均听阈无显著性差异,亦未见明显的内耳病理形态学改变。免疫后B、C、D、E组血清特异性抗体水平显著升高。免疫后B组未发现听损动物,C组3只动物(4耳)、D组4只动物(6耳)、E组4只动物(6耳)出现听损,部分听损耳出现膜迷路积水和螺旋神经节细胞减少。免疫组化显示68 000蛋白抗体主要分布于血管纹,Corti器也有少量分布;28 000蛋白抗体主要分布于螺旋神经节;42 000和58 000蛋白则在螺旋神经节、血管纹和螺旋韧带部位均有分布。结论: 粗制内耳抗原中42 000、58 000和68 000亚组分均能独立诱发自身免疫性梅尼埃病样内耳病变,实验性自身免疫性梅尼埃病的致病内耳抗原组分可能并非一种。

Objective To investigate whether or not that the four kind of components pured from conspecific inner ear membranous labyrinth tissue immunize guinea pig and induce experimental autoimmune Meniere's disease.At the same time,we would like to determine these antigen's distributing and expressing position in inner ear.Methods 70 guinea pigs were respectively divided into group B,group C,group D and group E and were immunized respectively with components of inner ear tissue antigen (28 000,42 000,58 000 and 68 000 proteins).Auditory brainstem response(ABR),level of specific antibody against every component of inner ear antigen levels in serum,vestibular function and pathohistological morphological changes of inner ear were observed.The distribution of protein antigen and expressing position in inner ear were located with immunohistochemistry observation.Results After immunization,there were no significant changes of hearing threshold in group A and group B,also no inner ear morphologic changes were found in A and B group.The levels of specific antibodies in serum of group B,C,D and E groups increased significantly after immunization.No animals were found to have hearing loss after immunization in group B,3 animals (4 ears) in group C,4 animals (6 ears) in group D and 4 animals (6 ears) in group E were found to have hearing loss.Labyrinthine hydrops and decrease of spiral ganglion cells were found in some animal ears of group C,D and E,all these ears were found to have hearing loss.The 68 000 protein mainly distributed at stria vascularis and small the Corti's organ;The 28 000 distributed strictly in the spiral ganglion;The 42 000 and 58 000 protein widely distributed in membranous labyrinth,including the spiral ganglion,Corti's organ,stria vascularis and spiral ligament.Conclusion Three components of the inner ear tissue antigen,the 42 000,58 000 and 68 000 could respectively induce autoimmune Meniere's disease,which means more than one components of inner ear antigens are responsible for the experimental AIMD.
