俯卧位辅助治疗早产儿呼吸暂停的病例对照研究

摘要：

目的: 探讨俯卧位辅助治疗早产儿呼吸暂停的疗效。方法: 将于新生儿重症监护室(NICU)住院的84例患儿随机分为治疗组(41例)和对照组(43例)。采用俯卧位头侧位体位疗法,头部提高15°,每天俯卧位时间≥16h,其它治疗方法与对照组相同,均采用多功能监护仪监护,设置呼吸暂停报警。临床疗效分为显效、有效和无效。结果: 经Ridit分析两组差异有统计学意义。治疗组呼吸暂停发作次数、持续时间、消失时间均短于对照组(均P<0.05)。结论: 俯卧位对早产儿呼吸暂停的疗效肯定,可以在NICU中推广应用。

Objective To investigate the therapeutic effects of prone position on apnea of preterm infants.Methods Eight-four patients divided into two groups randomly,treatment group(41)and control group(43),were admitted to neonatal intensive care unit(NICU) in October,2005 to October,2007.In the treatment group,babies were kept in prone position at least 16 h a day,and head was raised for 15 degree up.Other therapies in the two groups were in the same. Episodes of apnea were monitored by multifunction monitor.Clinical outcome were recorded as excellent,effective,in effective.Results The therapeutic effect of treat group was better than that of control group.Episodes of apnea,duration and extinction time were significant statistically in two groups (P<0.05).Conclusion Prone position is helpful to maintain the stable pattern of breath in preterm infants.The application of prone position for apnea of preterm infants may be extended in NICU.
