细胞凋亡在呼吸机相关性肺损伤中的作用

摘要:

目的:观察细胞凋亡在呼吸机相关性肺损伤(ventilator induced lunginury,VILI)中的作用,探讨呼吸机相关性肺损伤发生的机制;方法:将50只健康雄性Wistar大鼠随机分为对照组(插管后不进行机械通气)及机械通气组(插管后机械通气:潮气量20ml/kg,频率为50次/min,吸呼比为1∶3, FiO2为21%,通气时间:5h),观察实验前后动脉氧分压变化;采用Annexin-V/PI双染方法检测肺组织及肺泡灌洗液细胞凋亡;取各组大鼠左全肺称湿重,置于烘箱内80℃干烤24h后称干重,并计算肺组织湿/干重(W/D)比值;制备肺组织切片进行病理学观察;结果:两组大鼠动脉氧分压无显著差异(p>0.05),机械通气组肺组织及肺泡灌洗液细胞凋亡明显增加(p<0.01),机械通气组肺组织湿/干重(W/D)比值显著高于对照组(p<0.01),机械通气组肺组织病理切片可见肺组织水肿;结论:机械通气可导致肺损伤,肺组织细胞凋亡可能在其发生、发展过程中起重要作用.

Objective: To investigate the role of apoptosis in ventilator induced lunginury (VILI). in the role of ventilator-associated lung injury mechanisms; Methods: 50 healthy male Wistar rats were randomly divided for the control group (without mechanical ventilation after intubation) and mechanical ventilation group (after intubation mechanical ventilation: tidal volume 20ml/kg, frequency of 50 times / min, respiratory absorption ratio of 1:3, FiO2 21%, ventilation Time: 5h), observed before and after experimental changes in arterial partial pressure of oxygen; used Annexin-V/PI detected by double staining in lung tissue and alveolar lavage fluid cell apoptosis; get rats in each group, said the left whole lung wet weight, placed in oven within 80 ℃ said after 24h dry roasted dry weight, and calculate the lung tissue wet / dry weight (W / D) ratio; Preparation of lung tissue biopsies pathological examination; Results: The two groups of rats were no significant differences in arterial partial pressure of oxygen (p "0.05), mechanical ventilation and alveolar lavage fluid in lung tissue significantly increased apoptosis (p <0.01), mechanical ventilation in lung tissue wet / dry weight (W / D) ratio were significantly higher (p <0.01) , mechanical ventilation can be seen in lung tissue biopsy lung tissue edema; Conclusion: Mechanical ventilation can cause lung injury, lung cell apoptosis may, in its genesis and development play an important role.
