螺内酯预处理对兔非梗死区冠脉内皮eNOS表达及心梗后血流动力学的影响

摘要: 目的:探讨螺内酯对兔心梗早期非梗死区冠脉内皮eNOS表达及血流动力学的影响。方法:将24只雄性新西兰大耳白兔随机分为A(n=8)、B(n=8)、C(n=8)三组,A、B二组均予高脂饲喂四周,A组同时给予螺内酯(20mg/kg/d)口服四周。A、B组结扎前降支复制急性心肌梗死模型,C组穿线但不结扎。测定术前及术后15分钟的血醛固酮(Aldo)浓度及结扎近心段冠脉内皮型一氧化氮合成酶(e-NOS)表达情况。经颈动脉插管测定左室内的平均收缩压(MSP)、平均舒张压(MDP)、平均动脉压(MAP)及左室收缩/等容舒张功能(+dp/dt max、-dp/dt max)的变化并加以比较。结果:非梗死区冠脉e-NOS表达情况:A、B组均弱于C组,而A组高于B组(P均<0.05);A、B二组术后Aldo浓度明显高于C组(P<0.05),A、B组间比较差异无显著性;A、B二组左室MSP、MDP、MAP及+dp/dt max的变化值明显高于C组(P<0.05),而A组左室的MSP、+dp/dt max的变化值又均高于B组(P<0.05)。结论:螺内酯可使非梗死区冠脉内皮e-NOS表达增强,高脂喂养减弱这一表达;螺内酯可以改善心梗早期的血流动力学状态,具体机制可能与其能协同Aldo的非基因组作用共同促进心肌非梗死区冠脉扩张,增加心肌灌注有关。

Abstract: Objective: to investigate the effect of spironlactone on the expression of endothelial nitric oxide synthase(e-NOS) of rabbit coronary artery in the area of noninfarcted and hemodynamics of cardiovascular in early period of acute myocardial infraction(AMI). Methods:24 male rabbits were divided into three groups(A,B,C)randomly. Rabbits in group A(n=8) and B(n=8) were both feeded with high fat diet for four weeks, and those in group A were feeded with spironolactone(20mg/kg/d)at the same time. The middle and distal segment of anterior descending branch of all rabitts in group A and B were ligated to make AMI model,and animals in group C weren’t deligated. The level of aldosterone (Aldo) and expression of endothelial nitric oxide synthase (e-NOS) in coronary endotheliocyte which located near the spot of deligation were determinated fifteen minutes postoperation. Mean systolic pressure(MSP) ,mean diastolic pressure(MDP), mean aterial blood pressure(MAP) and isovolumic contraction / relaxation function(+dp/dt max、-dp/dt max)of left ventricular were measured and compared, by the way of making carotid arterial cannulation. Results: the expression of e-NOS in coronary endotheliocyte that hadn’t been ligated in group A and B were both more weaker than that in group C, and that in group A were better than that in group B(P<0.05). The level of Aldo in group A and B was much higher than that in group C(P<0.05),and there were no significant difference between than that in group A and group B. The change of MSP、MDP、MAP and +dp/dt max of left ventricular in group A and B were much higher than that in group C, and the changes of MSP and +dp/dt max of left ventricular in group A were higher than that in group C. Conclusion: Spironolactone could reinforce the expression of e-NOS in coronary endotheliocyte that hadn’t been deligated, and high fat feeding could decrease it. Spironolactone could improve the hemodynamics state of heart in pristine period of AM
