阻塞性睡眠呼吸暂停低通气综合征患者血浆apelin水平的变化及其意义

摘要:

目的:探讨Apelin与OSAHS及其严重程度之间的关系。方法:选取从2008年1月至2009年12月期间在我院睡眠呼吸监测中心行睡眠呼吸监测的患者,根据其呼吸暂停低通气指数(AHI)分为OSAHS组(89例)及非OSAHS组(30例),各组再根据BMI分为肥胖、超重、正常体重组,同时OSAHS组中肥胖、超重、正常体重各亚组又根据AHI分为轻、中、重度三组。所有受试者均接受多导睡眠仪监测,并采用酶联免疫法测定血浆apelin(ng/ml)的含量。结果:非OSAHS组及轻、中度的OSAHS组中,肥胖者血浆apelin水平明显高于超重者,超重者又明显高于正常体重者;在重度OSAHS组间,肥胖、超重、正常体重各组间apelin含量差异无统计学意义;各程度的OSAHS患者血浆apelin水平均明显高于相应体重的无OSAHS者,中、重度OSAHS患者血浆apelin水平明显高于轻度OSAHS组,中、重度OSAHS患者血浆apelin水平无差异。血浆apelin水平与AHI、BMI、颈围呈正相关(r值分别为0.581、0.392、0.432,P值均<0.001),与LSaO2、MSaO2呈负相关(r分别为-0.320、-0.495,P 值均<0.01)。结论:肥胖和OSAHS均可以引起apelin水平的增高,apelin与OSAHS及其严重程度密切相关。

Objective: To investigate the relationship between Apelin and OSAHS. Method: According to apnea-hypopnea index (AHI) and body mass index (BMI), We’ve selected the OSAHS group (89 cases) and non-OSAHS group (30 cases), from the patients ongoing monitoring of sleep apnea in the sleep disorder center in ZhongDa Hospital from January 2008 to December 2009. According to BMI, the group of data is divided into obese, overweight, normal weight, in the mean time, each obese, overweight, normal weight sub-group of OSAHS group is divided into mild, moderate and severe three ones according to AHI. All subjects received polysomnography monitoring, and plasma apelin (ng / ml) levels using enzyme-linked immunoassay. Results :In the non-OSAHS group and mild to moderate OSAHS group, the obese plasma apelin levels were significantly higher than overweight, the overweight were also significantly higher than the normal weight ones. Yet in the severe OSAHS group, there was no significant difference among the obesity group, overweight group and the normal weight group (P> 0.05). OSAHS groups were significantly higher than the groups without OSAHS of the same weight degree. Plasma concentration of apelin was significantly higher in patients of moderate and severe OSAHS groups than in the mild OSAHS group. But there is no significant difference been explored between the moderate OSAHS group and severe OSAHS group. Plasma apelin level was significantly correlated positively with AHI (r=0.581, P <0.01) and BMI (r=0.392, P <0.01), but correlated negatively with LSaO2 (r=-0.320, P <0.01) and MSaO2 (r=-0.495, P <0.01). Conclusion : The plasma apelin levels increased in OSAHS patients and obese patients . Apelin may play an important role in the occurrence and development of OSAHS.
