腔内修复术与传统开放术对破裂腹主动脉瘤的临床疗效比较

摘要：

目的：比较开放性手术（OSR）和腔内修复术（EVAR）治疗破裂腹主动脉瘤（RAAA）的近期临床疗效。方法：观察32例RAAA患者（其中EVAR组13例、OSR组19例）的手术时间、术中失血量及输血量、内漏发生率、术后死亡率、高腹压发生率、二次有创干预率。结果：在手术时间、术中输血输液量、住院时间、术后30 d死亡率、不良事件发生率、二次干预率等方面，EVAR组明显低于OSR组（P<0.05）。但EVAR组高腹压及内漏发生率较高。结论：EVAR救治RAAA的临床疗效比OSR更具有优势。

Objective: To compare the short-term clinical efficacy of open surgery(OSR) with that of endovascular repair(EVAR) in the treatment of ruptured abdominal aortic aneurysm(RAAA).Methods: There were 32 cases of RAAA with 13 cases in group EVAR and 19 in group OSR. The operation time, blood loss, blood transfusion, incidence of internal leakage, postoperative mortality, high abdominal pressure incidence, and twice invasive intervention rates were observed in the two groups.Results: The transfusion volume, hospitalization time, mortality rate 30 days after operation, incidence of adverse events and twice intervention rate of EVAR group were significantly lower than those of OSR group(P<0.05). But the incidences of high abdominal pressure and internal leakage were higher in the EVAR group.Conclusion: The clinical efficacy of EVAR in the treatment of ruptured abdominal aortic aneurysm is better than that of OSR.
