丹参川穹嗪联合甲钴胺对早期DPN患者血液流变学及炎症因子水平的影响

摘要：

目的：探讨丹参川穹嗪联合甲钴胺对早期糖尿病周围神经病变（DPN）患者血液流变学及炎症因子水平的影响。方法：将2012年3月至2015年8月经我院诊治的153例早期DPN患者采取双盲法随机纳入A、B、C 3组，每组51例，在保证3组患者严格控制血糖前提下，A组给予甲钴胺静脉滴注治疗，B组给予丹参川穹嗪注射液静脉滴注，C组给予丹参川穹嗪联合甲钴胺静脉滴注治疗，疗程2周，比较3组患者在治疗效果、神经病变自觉症状问卷（total symptoms score，TSS）、血液流变学、炎症因子水平等方面差异。结果：治疗后3组患者症状有所改善，总有效率C组（96.1%）高于A组（72.5%）及B组（66.7%）；TSS评分C组（7.69±1.73）低于A组（9.48±1.72）及B组（9.95±1.92），差异均具有统计学意义（P<0.05）。C组血液流变学改善效果优于A、B组，C组炎症因子表达水平低于A、B组，差异均具有统计学意义（P<0.05）。结论：丹参川穹嗪联合甲钴胺可提高DPN治疗效果，改善神经功能、降低血液黏度并下调部分炎症因子蛋白表达水平，效果优于单用甲钴胺。

Objective: To investigate the effects of Salvia TMZ joint mecobalamin on hemorheology and inflammatory cytokine levels in patients with early diabetic peripheral neuropathy(DPN). Methods: 153 patients with early DPN on March 2013-Augest 2015 in our hospital were randomly included in the group A, B, C, and each group had 51 cases. Ensuring patients of the three groups could be strict control of blood glucose premise, group A was given intravenous methylcobalamin treatment, group B received Salvia TMZ injection, group C administered Salvia TMZ joint mecobalamine intravenous. After 2 weeks of treatment the difference of three groups in treatment effect, neuropathy main feel symptom questionnaire total symptoms score(TSS), rheology, and expression of inflammatory factors were compared. Results: After treatment, the symptoms have improved in the patients of three groups, and on the overall efficiency in the treatment, group C(96.1%) was higher than group A(72.5%) and group B(66.7%); on the TSS score, group C(7.69±1.73) was lower than group A(9.48±1.72) and group B(9.95±1.92), the difference was statistically significant(P<0.05); In blood rheology, group C was improve better than group A and group B, while on the level of expression of inflammatory factor, group C was lower than group A and group B, the differences were statistically significant(P<0.05). Conclusion: Salvia TMZ joint mecobalamine can improve treatment effect of diabetic peripheral neuropathy, improve nerve function, reduce blood viscosity, and lower part of the inflammatory cytokine protein levels, the effect is better than single mecobalamin.
