X线、超声、磁共振成像多技术联合鉴别乳腺良恶性肿块的价值研究

摘要:

目的:评估X线、超声、磁共振成像联合鉴别乳腺良恶性肿块的价值。方法:收集2011年1月至2016年1月在本院有完整影像资料患者79例,其中38个为小病灶(病灶直径≤2 cm)。所有患者均行乳腺X线、超声及磁共振成像检查,诊断结果与病理对照。所有数据输入SPSS 19.0统计软件。采用χ2检验比较X线、超声和磁共振成像三者联合与X线和超声两者联合敏感性和特异性的差异。采用χ2检验比较X线、超声和磁共振成像对乳腺小病灶的检出率。结果:(1)X线和超声两者联合的敏感性和特异性分别为94.9%和75.4%,AUC为0.852;X线、超声和磁共振成像三者联合的敏感性和特异性分别为97.4%和75.4%,AUC为0.858。X线、超声和磁共振成像三者联合与X线和超声两者联合敏感性和特异性的差异均无统计学意义。(2)X线、超声及磁共振成像对乳腺小病灶的检出率分别为68.4%、70.1%和92.1%。结论:(1)与X线和超声两者联合相比,X线、超声和磁共振成像三者联合在鉴别乳腺良恶性肿块的敏感性和特异性上并未表现出优势。(2)磁共振成像对乳腺小病灶的检出能力均高于X线和超声。

Objective:To assess differentiation of benign between malignant breast tumors by a combination of mammography,ultrasonography,multimoldality MR imaging. Methods: 79 patients including 38 small lesions were enrolled during Jan 2011 to Jan 2016(Lesion diameter ≤2 cm regarded as small lesions), which were all underwent mammography, ultrasonography, multimoldality MR imaging. Then all data were compared with pathologic results. Statistical analyses were carried out using SPSS19.0. χ2 test and ROC were used to analysis the sensitivity and specificity. The detection rate of small breast tumor of the three methods was also using χ2 test. Results:(1) The combination of mammography and ultrasonography had 94.9% sensitivity, 75.4%specificity and 0.852AUC.While the combination mammography, ultrasonography and multimoldality MR imaging had 97.4% sensitivity, 75.4% specificity and 0.858 AUC. Both of specificity and sensitivity showed no statistical significance between digital mamagraphy combination ultrasonography and mamagraphy, ultrasonography combination multimoldality MR imaging in differentiating malignant and benign lesions. (2) The detection rates of small lesions with mammography, ultrasonography and multimoldality MR imaging were respectively 68.4%, 70.1%, 92.1%. There was no significant difference between mammography and ultrasonography. The detection rate of multimoldality MR imaging was higher than mammography and ultrasonography. Conclusion:(1) In conclusion, the combination of mammography and ultrasonography has no significantly better accuracy of breast tumor diagnosis than a combination of mammography,ultrasonography and multimoldality MR imaging. (2) The detection ability of multimoldality MR is higher than mammography and ultrasonography in small breast tumor. For multicentric and multifocal lesions, the detection ability of multimoldality MR imaging is unmatched by other imaging methods.
