冠心病合并2型糖尿病患者血清BDNF的浓度变化及临床意义

摘要:

目的:探讨脑源性神经营养因子(BDNF)在冠心病合并2型糖尿病患者血清中的浓度及临床意义。方法:依据病史及冠状动脉造影结果,将2013年8月至2015年8月在东南大学附属中大医院心血管内科就诊的患者240例分为3组:(1) 正常对照组(Con组),83例;(2) 单纯冠心病组(CAD组),85例;(3) 冠心病合并2型糖尿病组(CAD+DM组),72例。使用酶联免疫吸附测定法(ELISA)测定纳入对象的血清BDNF浓度,采用Gensini评分测定冠状动脉的狭窄程度。结果:(1) CAD组和CAD+DM组血清的BDNF浓度较Con组明显升高(P<0.01);CAD+DM组血清的BDNF浓度高于CAD组(P<0.01)。(2) CAD+DM组的Gensini评分高于CAD组(P<0.01);血清BDNF浓度与Gensini评分呈正相关。结论:(1) 冠心病合并2型糖尿病患者的血清BDNF浓度显著高于CAD组及Con组;(2) 血清BDNF浓度与冠脉狭窄程度Gensini评分呈正相关,提示BDNF可在一定程度上反映冠状动脉的狭窄程度。

Objective:To evaluate the expression of brain-derived neurotrophic factor(BDNF) in serum of patients with coronary artery disease(CAD) with type Ⅱ diabetes and its clinical value. Methods:240 patients were enrolled in our research, who were admitted in the Department of Cardiology of the Zhongda Hospital Affiliated to Southeast University from August 2013 to August 2015. According to medical history and the results of coronary angiography(CAG), the patients were divided into 3 groups as follows:normal control group(Con):n=83;(2) CAD group:n=85;(3) CAD combined type 2 DM(CAD+DM):n=72. The levels of serum BDNF were checked by using enzyme-linked immunosorbent assay(ELISA), the severity of coronary artery lesion was evaluated according to the Gensini score. Results:(1) Plasma BDNF level was significantly higher in CAD, CAD+DM group than that in Con group(P<0.01), and plasma BDNF level in CAD+DM group was significantly higher than that in CAD group(P<0.01); (2) The patients in CAD+DM group had a higher Gensini score than those in CAD group; the serum level of BDNF was positively correlated with Gensini score. Conclusion:(1) The plasma BDNF level in the CAD+DM group is higher than that in the CAD group and in the Con group; (2) The plasma BDNF with Gensini scores is positively correlated, which suggests that the plasma BDNF level may reflect coronary artery stenosis to some extent.
