摘要：

目的:探讨小儿主动脉弓离断(IAA)的诊断方法.方法:分析6例小儿IAA病例的临床特征,讨论其临床分型及诊断方法.结果:6例患儿均合并动脉导管未闭及肺动脉高压,患儿上下肢血压及血氧饱和度差异有助于本症的初步诊断,二维超声心动图可进行初步诊断筛查,准确诊断主要依赖心血管造影、MRI及电子束CT检查.结论:二维超声心动图结合电子束CT检查是诊断小儿IAA比较简便、安全及准确的方法.

Objective To explore a better way to diagnose interruption of aortic arch(IAA) in children.Methods The clinical features, the clinical classification and diagnostic methods of 6 cases from 1985 to 2002 were retrospectively analyzed. The diagnoses were confirmed either by cardioangiograph(CAG) or operation.Results The results showed that all the cases in this group with the complex deformity of large patent ductus arteriosus represented severe pulmonary hypertension. Interruption of aorta arch could be located according to the difference of blood pressures on their arms and legs. Two dimensional echocardiography (2DE) scan was used as a screening test,and CAG, magnetic resonance imaging (MRI), electric beam CT could accurately diagnose IAA.Conclusion 2DE combined with electric beam CT is comparatively a easy, safe and accurate method of diagnosing IAA.
