缬沙坦对冠心病患者血管内皮功能的影响

摘要:

目的:探讨缬沙坦对冠心病患者血管内皮功能的影响.方法:将42例冠心病患者随机分为常规治疗组(阿司匹林与倍他乐克)和缬沙坦组(常规治疗加缬沙坦),利用超声诊断仪检测肱动脉内皮依赖舒张性变化率(FMD),并检测治疗前后患者血浆中一氧化氮(NO)、脂质过氧化物代谢产物(MDA)、超氧化物歧化酶(SOD)、低密度脂蛋白(LDL-C)水平的变化.结果:与常规治疗组相比,缬沙坦治疗后患者FMD明显增加(P<0.05);血清MDA水平显著降低(P<0.05),NO与SOD 水平显著升高(P<0.05), LDL-C浓度无显著差异 (P>0.05).结论:缬沙坦通过增加冠心病患者血浆中NO水平并增加SOD产物而清除MDA,从而提高NO的生物利用度.此效应有助于改善冠心病患者血管内皮功能.

Objective  To observe the effect of valsartan on function of vascular endothelium in patients with coronary heart disease (CHD).Method  42 cases with CHD were randomized to the control group (treated with betaloc 50?mg·d    -1 and aspirin 75?mg·d    -1) and the valsartan group(treated with betaloc 50?mg·d    -1,aspirinin 75?mg·d    -1 and valsartan 100?mg·d    -1). After treatment for 6 weeks,endothelium-dependent relaxing  function was determined by high-resolution ultrasound. The level of plasma NO,MDA and SOD activity as well as LDL?C were determined after and before treatment.Result  After therapy with valsartan,there was more significantly increased FMD in losartan group than the control group (P&lt;0.05). The concentration of plasma MDA was lowered (P&lt;0.05); the level of plasma NO and SOD were increased significantly (P&lt;0.05), while the concentration of plasma LDL?C showed no change (P&gt;0.05).Conclusion  Valsartan may exert anti-oxidative stress by increasing the level of NO and SOD, therefore increase the bioavailability of NO,which contribute to the improvement of the function of vascular endothelium in patients with CHD.
